Private Coaching Agreement

Welcome
Thank you for choosing Life By Design Coaching. This agreement outlines our coaching relationship, expectations, and responsibilities so we can work together in a respectful, productive, and supportive environment.
By signing below, both Coach and Client agree to the following terms.
Coaching Relationship
The Coach agrees to provide professional nurse coaching services as outlined in the Coaching Package attached as Attachment A.
Coaching is a collaborative process designed to help clients clarify goals, increase self-awareness, overcome obstacles, and create meaningful, lasting change.
The Client understands that coaching:
- Is not medical treatment.
- Is not psychotherapy or counseling.
- Is not a substitute for medical or mental health care.
- Does not diagnose, prescribe, or treat disease.
If the Client is currently under the care of a physician, therapist, or other licensed healthcare provider, the Client agrees to continue that care while participating in coaching.
For clients residing in states where the Coach does not hold an active nursing license, the Coach is providing services solely as a professional coach and is not acting in the capacity of a registered nurse.
Coaching Methodology
The Coach may utilize various evidence-informed coaching approaches, including motivational interviewing, appreciative inquiry, strengths-based coaching, mindfulness practices, reflective questioning, accountability strategies, and wellness education.
The Client understands that participation is voluntary and that results depend upon the Client’s own effort and commitment.
No guarantees are made regarding specific outcomes.
Scheduling
Sessions are scheduled by mutual agreement.
Clients may reschedule appointments with at least 24 hours’ notice.
Appointments cancelled with less than 24 hours’ notice or missed without notice will be considered completed and cannot be rescheduled.
Payment
Payment may be made in full or through an agreed-upon payment plan. For monthly plans, payments are due on the first day of each month and will be processed automatically for the duration of the program. Coaching sessions will be provided only when payments are current.
Invoices unpaid more than ten (10) days after the due date may lead to suspension or termination of coaching services.
